Participant Waiver

By signing and submitting this document, I

  • confirm that I have registered for the Active Lifestyle event through Returned & Services League of Australia (Queensland Branch) (RSL Queensland);
  • authorise RSL Queensland to share my personal details with the event manager for the purposes of booking and registration;
  • understand that the event manager is responsible for the administration and delivery of the Active Lifestyle event and RSL Queensland are in no way responsible for participant health and safety or for the administration and delivery of the Active Lifestyle event;
  • voluntarily choose to participate in the program activities;
  • acknowledge that I am undertaking the Active Lifestyle event solely at my own risk;
  • release RSL Queensland from any claims or demands arising out of or in connection with my participation in the Active Lifestyle event as well as any damage, loss, or expense howsoever caused which may arise as a result of my participation in the Active Lifestyle event;
  • understand that the difficulty of this program is categorised as Green and I am confident in my ability to partake in this level of physical activity (please see registration form for further information);
  • confirm that I will abide by the rules and requirements of participation outlined by the event manager;
  • acknowledge that I will be unable to participate in the Active Lifestyle event if I do not comply with the rules and requirements of participation outlined by the event manager and RSL Queensland will not be responsible for any loss or damage suffered by me as a result of me being unable to participate;
  • acknowledge that I have read the information set out in the Talent Release Details overleaf and agree to my personal information to be used by RSL Queensland. I will notify RSL Queensland if I have any objection to this; and
  • consent to give and receive this document, including signatures to execute this document, by electronic communication in accordance with the Electronic Transactions (Queensland) Act 2001 (Qld).
