- confirm that I have registered my child, or an individual for whom I have authorised decision making responsibility, for the Event through Returned & Services League of Australia (Queensland Branch) (RSL Queensland);
- authorise RSL Queensland to share my child's, or an individual for whom I have authorised decision making responsibility's personal details with the event manager for the purposes of booking and registration;
- confirm that my child, or the individual for whom I have authorised decision making responsibility, chooses to participate in the program activities;
- acknowledge that my child, or an individual for whom I have authorised decision making responsibility, is undertaking the Event solely at my own risk;
- release RSL Queensland from any claims of demands arising out of or in connection with my child's, or an individual for whom I have authorised decision making responsibility, participation in the Event as well as any damage, loss, or expense howsoever caused which may arise as a result of my participation in the Event;
- understand that the difficulty of this program is categorised as medium (requires good physical ability and fitness) and I am confident in my child's, or an individual for whom I have authorised decision making responsibility's, ability to partake in this level of physical activity (please see registration form for further information);
- confirm that my child, or an individual for whom I have authorised decision making responsibility, will abide by the rules and requirements of participation outlined by the event manager;
- acknowledge that my child, or an individual for whom I have authorised decision making responsibility, will be unable to participate in the Event if I do not comply with the rules and requirements of participation outlined by the event manager and RSL Queensland will not be responsible for any loss or damage suffered as a result of my child, or an individual for whom I have authorised decision making responsibility being unable to participate; and
- consent to give and receive this document, including signatures to execute this document, by electronic communication in accordance with the Electronic Transactions (Queensland) Act 2001 (Qld).

We are seeking your consent to collect, use and disclose;
- your photograph
- a digital recording, audio recording or video recording of you
- information to describe your image, such as your gender, age, features or clothing worn
- information about your experience with our programs, services, events or activities

How will your personal information be used?
We may use your personal information (including, if you consent, your name and where you are from) (the media) in any of the following:
- printed publications such as brochures, booklets, fact sheets, newsletters, magazines and reports
- display banners and posters
- television, radio and newspaper advertisements and other media
- internet pages (such as our websites rslqld.org and rslartunion.com.au and/or RSL Queensland social media pages).
We may also share the media with affiliated organisations which share similar objects, such as:
- Mates4Mates Limited ACN 160 646 999

- any State or Territory Branch of the Returned & Services League of Australia Ltd
- any Sub Branch or District Branch within the jurisdiction of RSL Queensland

Modification of withdrawal of consent
When giving your consent you should be aware that any information published on websites and social media will be accessible to people worldwide, that the information will be indexed by search engines and that the information may be copied and used by any web user. This means that once the media is published on the internet, we will have no control over its subsequent use and disclosure.
If you wish to limit the consent given under this form, you must provide details of this in writing by attaching a signed letter to this consent form.
You can modify or withdraw your consent in writing at any time. However, any changes to the user of the media will only apply from the date we receive your withdrawal or modification of the consent.
We will make all reasonable efforts to remove the media from online material; however once the media is published the likelihood of its being completely removed at a later date is unlikely. For hard copy materials, we will remove the media from any reprints or future materials. Any existing hard copy material in which the media is used will not be withdrawn from use and publication.
Consent and release

- I assign to RSL Queensland the ownership of any copyright I own or may acquire in the media, and grant RSL Queensland a royalty free licence to use any other copyright material of mine, to the extent necessary to allow RSL Queensland to exercise its rights in the media.
- I consent to any and all acts or omissions, whether occurring before or after the giving of this consent that may otherwise infringe any moral rights in the media
- I consent to the media being reproduced, published or otherwise used by RSL Queensland in various forms, including but not limited to reports, newsletters, websites, films, publications, productions and presentations.
- Release and waiver
- I waive any rights, claims or interest I may have to control the use of the media in connection with the project or future projects and I release RSL Queensland in relation to the same.
- Undertakings
- I understand that by giving consent, RSL Queensland can use the media in publications, websites, productions and presentations for the purpose. RSL Queensland may reproduce, in whole or in part, the media in any and all media and methods of communication to the public anywhere in the world, whether the media is now known or invented in the future. RSL Queensland may provide the media to organisations which share similar objects to RSL Queensland who will use the media in accordance with their privacy policy.
- RSL Queensland may also license the media to any other organisation as we see fit.
- I warrant that if the media includes any copyright material belonging to other people, I have permission from the copyright owners to use, and to allow RSL Queensland to use, that material in accordance with this consent.
- I understand that RSL Queensland:
- will not pay me for giving this consent or for the use of the media
- may keep the media
- may use the media for the purpose
